Hi Sarah - Thanks for posting this. Just for the sake of clarity, what topics/areas are you looking for the survey to cover?

@Andrew Schulman We developed an exit survey to help us understand the quality of the services we offer. Most are single select answers but we do also offer free form replies. I was just curious to see a few others to see if we missed anything. Thanks.

There are other items in our too about spending down funds, cycling off to become their own 501c3 etc… but I’m just sharing the survey questions about our “service” (I am aware this is not the best term to use, but it’s a carryover we have yet to edit!)

Thank you Shava. We developed a survey focused on service, but I wanted to compare it with others to ensure we have what we need. I appreciate you sharing. we have quite a few choice options, one being a list of our services and a question about how their satisfaction for each (very satisfied, somewhat satisfied, neither satisfied nor dissatisfied, somewhat dissatisfied, very dissatisfied, did not use). And we have a few free form fields.
